• NDE Application (for 1st-8th graders)
• Student Evaluation for incoming 1st graders (to be filled out by child's current teacher)
• Student Evaluation for incoming 2nd-8th graders (to be filled out by child's current teacher)

First grade applicants for the 2011-2012 school year:

• must be born before September 1, 2005
• must take the new student entrance test

Second-sixth grade applicants for the 2011-2012 school year:

• must submit a copy of their current report card
• must submit test scores (Iowa Test, STAR, etc.)
• must take the new student entrance test

Please note: Submission of the application form does not guarantee entrance to NDE. Parents of applicants will be notified after the faculty and principal have evaluated the applicant's records. NDE is a Catholic School; priority for admission will be given to Catholic children.
